CMS Proposes Big Changes to Improve Medicare Outcomes

👏 The shift from reactive to proactive care is finally getting federal support!
The Centers for Medicare & Medicaid Services just proposed the Ambulatory Specialty Model (ASM), a bold new payment model designed to improve care for patients with heart failure and low back pain, two of Medicare’s highest-cost conditions.
Set to launch in January 2027, ASM has the goal of: ✅ Reducing avoidable hospitalizations ✅ Improving outcomes & patient experience ✅ Lowering Medicare costs ✅ Rewarding providers for upstream chronic disease management
At Vironix Health, this isn’t just a proposal - it’s our daily practice.
We already support patients with chronic heart failure (CHF) through our Virtual Care Management (VCM) programs, helping cardiologists and primary care physicians deliver proactive, high-quality care.
HERE’S HOW: 📲 Remote monitoring tools: We provide cellular-enabled BP cuffs & weight scale (no apps or Wi-Fi needed) 🧠 AI-powered risk alerts: Clinically validated algorithms flag patient deterioration in real-time 👩⚕️ Care team integration: Our nurses & MAs work as an extension of the physician team 🚑 Preventive intervention: We reduce ER visits and hospitalizations by catching issues early
The future of specialty care is proactive, tech-enabled, and patient-centered, and we’re proud to be building it now.
