CMS Proposes Big Changes to Improve Medicare Outcomes

The Centers for Medicare & Medicaid Services just proposed the Ambulatory Specialty Model (ASM), a bold new payment model designed to improve care for patients with heart failure and low back pain, two of Medicare’s highest-cost conditions.

Set to launch in January 2027, ASM has the goal of: ✅ Reducing avoidable hospitalizations ✅ Improving outcomes & patient experience ✅ Lowering Medicare costs ✅ Rewarding providers for upstream chronic disease management
